The main purpose of this position
The Registered Nurse, Supported Attachment provides direct patient care for unattached patients, completing all clinical intake, assessments, and Cumulative Patient Profiles (CPPs). The RN ensures patients are prepared for primary care attachment, supports equitable access, and collaborates with community partners and primary care clinicians.
Responsibilities
Patient Intake & Onboarding
- Conduct comprehensive patient intake assessments, including health history, social determinants of health, and completion of CPP documentation.
- Perform clinical assessments within RN scope (height, weight, blood pressure, etc.).
- Facilitate medication reconciliation in collaboration with pharmacists or primary care teams.
- Collect and document key patient information (emergency contacts, substitute decision makers, existing care team members).
- Prepare patients for initial primary care appointments, reviewing clinic processes, expectations, and next steps.
Navigation & Care Coordination
- Support patients in accessing community, social, and health system supports, limited to non-clinical navigation.
- Ensure access to language interpretation, cultural translation, and accessibility supports.
- Coordinate secure transfer of medical records in accordance with PHIPA.
- Facilitate warm handoffs to primary care clinicians and teams.
Collaboration & System Integration
- Work closely with PCN clinical leads and primary care clinicians to identify patients suitable for Supported Attachment.
- Collaborate with Care Connectors to support HCC waitlist management.
- Engage community partners to strengthen referral pathways.
- Align workflows with provincial programs (Health811, Self-Management Programs, Preventive Care, STOP Smoking).
Equity, Inclusion & Cultural Safety
- Apply equity-informed, trauma-informed, and culturally safe practices.
- Pay attention to FNIMUI, Francophone, newcomer, and other equity-deserving populations.
- Participate in required training, including Indigenous Cultural Safety and Active Offer of French Language Services.
Data Collection, Reporting & Quality Improvement
- Collect and maintain accurate program data for reporting.
- Track patient volumes and clinician participation.
- Contribute to workflow refinement and continuous quality improvement.
Knowledge and skills necessary to be successful in this role
- Registered Nurse (RN) in good standing with the College of Nurses of Ontario.
- 2–5 years experience in primary care, community health, or patient navigation.
- Strong clinical assessment, documentation, and communication skills.
- Experience with EMRs (e.g., PS Suite) and digital documentation systems.
- Commitment to equity, anti-racism, and trauma-informed care.
Working conditions
- Multi-site, community-based, and virtual work.
- Regular travel within Halton Region.
- Flexible scheduling to meet patient and clinic needs.
Job Types: Full-time, Fixed term contract
Contract length: 12 months
