<p><b>0.8 EFT</b> - 31 hours a week</p>
<p><b>HOURS OF WORK:</b> Days/Evenings</p>
<p><b>START DATE:</b> January 5, 2026 (or as mutually agreed)</p>
<p><b>TERM ENDING:</b> July 23, 2027 (Subject to 2 Weeks’ Notice)</p>

<h3>Qualifications</h3>
<ul>
<li>Minimum of 2 years experience in Community Health and/or Primary Health Care and/or Public Health and active registration in the College of Registered Nurses of Manitoba in the RN – Extended Practice Register</li>
<li>Demonstrated ability to work independently and collaboratively as a member of a multi-disciplinary team</li>
<li>Current CPR certification</li>
<li>Valid drivers license, access to a vehicle may be required</li>
<li>Knowledge of community resources</li>
<li>Working knowledge of basic computer usage and Microsoft Office</li>
<li>Demonstrated effective communication skills</li>
<li>Experience in development and evaluation in Health Promotion and Education programs as asset</li>
</ul>
<h3>Responsibilities</h3>
<h3>Assessment</h3>
<ul>
<li>Systematically obtains health histories, performs comprehensive health assessments and exams.</li>
<li>Identifies health needs and risks, establishes urgency and establishes plan of care.</li>
</ul>
<h3>Diagnosis</h3>
<ul>
<li>Orders, conducts and interprets diagnostic and relevant screening procedures.</li>
<li>Synthesizes data from health history, physical exam and diagnostic tests to formulate appropriate differential medical diagnoses.</li>
</ul>
<h3>Clinical Management</h3>
<ul>
<li>Manages common primary health care problems (acute minor illnesses and stable chronic conditions) within her/his scope of practice.</li>
<li>Provides information, education, support and advocacy to clients.</li>
<li>Follows current clinical practice guidelines.</li>
<li>In collaboration with the patient and physician as needed, discusses and identifies appropriate management approaches, including prescriptive treatment, non- prescriptive therapies and minor invasive procedures.</li>
</ul>
<h3>Evaluation</h3>
<ul>
<li>Identifies the need for care outside the scope of nurse practitioner practice and consults/ refers appropriately</li>
<li>Evaluates, reassesses and modifies plan of care as necessary and provides appropriate follow up using home visits, telephone calls, appointments and referrals as necessary.</li>
</ul>
<h3>Program/Agency</h3>
<ul>
<li>Charts appropriately in accordance with Klinic policies.</li>
<li>Fosters collaborative relationships with Klinic staff as is necessary to ensure an interdisciplinary approach to meeting health care needs.</li>
<li>Networks with other health care providers and professional agencies to enhance quality of care. Acts as an agency representative as requested</li>
<li>Assists in program development</li>
<li>Records statistics as required.</li>
<li>Assists with orientation, training and supervision of new staff and students.</li>
<li>Performs surveillance of health indicators and evaluates the effects of clinical outcomes and selected interventions.</li>
<li>Influences and contributes to the development of policies, protocols, education and research in the agency.</li>
<li>Contributes to the professional advancement of RN – EPs and educates the public about the role of RN – EPs.</li>
<li>Participates in training courses in order to maintain and improve quality of services.</li>
<li>Promotes team, program functioning and integration</li>
<li>Liaises with other service organizations and participates on external working groups/committees as required, conducts presentations promoting Klinic’s services.</li>
<li>Participate in formal self-evaluations by identifying strengths and limitations.</li>
<li>Seek appropriate supervision of own actions.</li>
<li>Performs other duties as may be assigned, not exceeding above stated qualifications</li>
