Reporting to the Director of Care, the Registered Practical Nurse – Behavioural Support Nurse is a critical role within our communities as it impacts the lives of residents.

What You'll Be Doing
You will be a welcomed member of the Sienna Senior Living team and impact in the following ways:
Provide sound clinical knowledge and skills, acting as clinical expert and resource
Provide comprehensive, quality resident and family-centered health care services
Aide in the transition of long term care residents with responsive behaviours between community, hospital and neighboring long-term care communities (LTCHs)
Consult on resident applications and make recommendations to meet the needs related to responsive/expressive behaviours
Screen and complete risk assessment for residents with new or change in behaviours
Coordinate referrals and engage community partners in supporting the resident care needs
Facilitate education sessions to build capacity in team members
Support the management of case reviews and clinical rounds
Engage the interprofessional team in the development of individualized plan of care
Who You Are
A Registered Practical Nurse in good standing the Ontario College of Nurses
Minimum 3 years' experience working in long term care
Demonstrated knowledge and skill working with older adults presenting with or at risk for responsive behaviours that may be associated with dementia, complex mental health, substance use and/or other neurological conditions
Specialized training in Gentle Persuasive Approaches (GPA), U-FIRST, P.I.E.C.E.S., Montessori, DementiAbility are an asset
You have good judgement, the ability to identify risk and can foster a positive work environment
You must be legally eligible to work in Canada or have a valid work permit
