We respect and value diverse life experiences and cultures and work to ensure that all voices are valued and heard.Reporting to the Manager of Integrated Services, the incumbent will be responsible for the following functions:Providing client needs identification through ongoing assessment and client directed care planningImplementing client centered plan of care that focus on strengths, wellness and empowerment to support recovery and goal achievementProviding crisis prevention and intervention planning with the hope of preventing a relapse and/or hospitalizationActing as a resource person or an advocate to assist individuals in obtaining information/access to services/supports which are deemed neededLiaising with family/significant other and natural supports to facilitate the clients’ links within the communityQualifications:Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse or Registered Practical Nurse with current registration with the C.NO. (preference to those with Canadian Certification in Psychiatric/Mental Health Nursing); Bachelor’s Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or a post-secondary degree with a relevant professional registration (acceptable to the employer);Experience and a detailed knowledge of serious mental illness, usually gained by several years of working with this populationValid Ontario Driver’s Licence and access to a vehicle are requiredHours of Work: 35 hours/week. Flexible hours are required to meet the position requirements.
Salary: Commensurate with experience and education within the range: $53,393 to $75,995 inclusive of HOOPP. Applicants should submit a covering letter and resume by September 18th, 2023
