Activity Coordinator – Richmond Care HomeJob Summary:As part of the care team at Richmond Care Home, the Activity Coordinator assists residents with activities that support their emotional, physical, and mental wellbeing. The Activity Coordinator reports to the Supervisor at Richmond Care Home, and supports the care team in monitoring the behaviours and needs of each resident.Primary Responsibilities include:Monitoring and Evaluation Uses information in the care plan, computer memos, and staff communication log when making observations, and seeks out guidance and information from the nurse, manager or other relevant staff members as required Reports all changes in client condition/situation in accordance with Carefor policies and proceduresPlanning Contributes to the development and revision of client’s service plans in collaboration with the charge nurse/Supervisor Sets service goals in collaboration with the client, the charge nurse and the Supervisor Any changes to the care plan must be implemented by the manager or nurse for safety, liability and financial reasonsImplementation· Performs tasks reflecting the needs of the client, based on the client care plan and other directives from charge nurse & supervisor. Tasks may include:o Providing personal support as directed by nurse or per client care plan; assistance in walking, climbing or descending stairs or eatingo Monitoring and maintaining resident and household safety; report to manager/ nurseo Recognizing the social and emotional needs of client and dealing with them effectively;o Meal set-up and assistanceo Light housekeeping/cleaningo Laundering of clothes and linens; laundry for incontinenceo Setting priorities for to effectively utilize time, energy, supplies and equipmento Completing assignments in the assigned length of timeo Help create and maintain a peaceful,’ normalized,’ home-like environment for guestso Suggest changes in Client Profile as necessaryo Ensure that the door remained locked at all timeso Assisting clients to keep their rooms clean and tidy and making bedso Encouraging and promoting resident independence (Organizational Resp.)o Treating residents with dignity and respect (Organizational Resp.)Communication Receives direction and assignments from the charge nurse or supervisor by phone, email or in person Observes the clients pattern of communication and relating and reports changes/concerns to the charge nurse or supervisor Relates to and shares information with other members of the clients health care team within and outside the organization in a professional and timely manner Reports verbally to the nurse any changes in the individual’s needs necessitating a change in the care plan Utilizes verbal and non-verbal communication skills effectively with the client Communicates with the clients respectfully and according to their needs and situation/condition Uses effective communication strategies to manage inappropriate, disruptive or aggressive behaviour Consults with the nurse or manager when unable to resolve difficulties with a client or team member or when requiring clarification about an issue Uses interpersonal skills to build caring/helping relationships with clients Acts as a team player, communicating effectively with all team membersHealth and Safety Functions and reports in compliance with the Occupational Health and Safety legislation, regulations, and Carefor policies and procedures Participates in health and safety training including WHMIS and applies this knowledge in the workplace Uses any/all protective equipment or clothing provided by the Carefor and does not interfere with protective devices so as to change or negate their functions Reports the absence of or defect in any equipment or protective device of which they are aware and which may endanger them self, another worker or the client Reports any violations of the Health and Safety Act or the regulations, or the existence of any hazard Does not use or operate any equipment, machine, device, or work in a manner that may endanger them self, other worker or any client Ensures all workplace incidents are reported in a timely manner in accordance with legislation, and Carefor policies and procedures for workplace injuries and incidents Identifies risks and potential health and safety risks for clients and other members of the health care team Dresses for work in such a manner as to ensure personal and client safetyQualifications include: Diploma, or Certificate in Recreation, Gerontology, Social Service Work or other related discipline Current Standard First Aid Current CPR – Level C Valid drivers licence and liability insurance an asset Dementia education/courses as asset Minimum of one year experience working with the seniors living with dementia, special needs or equivalent Organizing and running social & recreational activities Demonstrated ability to work with seniors requiring support Excellent communication skills, both verbal and written Excellent assessment skills Excellent Interpersonal skills Ability to take direction well Ability to work independently Ability to work in a team environmentCarefor welcomes applications from all qualified candidates.
