GENERAL SUMMARY:
Our clinic is comprised of a multi-disciplinary team of physicians and wellness experts who are here to help our patients live healthier, and more active lives.
The successful candidate will be responsible for assisting the team of Sports Health Providers, working with rehabilitation patients, to carry out corrective exercise programs and transition patients to ongoing training once the patient has completed the acute/chronic rehabilitation stage.
The successful candidate will be responsible for building their case-load and work with a variety of patients to provide exercise prescription for those requiring movement based re-education, sport-performance training, or exercise-based rehabilitation, and create and carry out corrective exercise programs that aim to help patients successfully transition back to work, function and activity. This candidate will also liaise with the Sports Rehab team for comprehensive collaboration to ultimately provide the best possible care and service to our patients.

How you will make a difference as an MVP Specialist on our team:
Strong skills in communication and collaboration among the MVP team and across multiple departments at Cleveland Clinic Canada.
Collaborate to plan patient care.
You will perform assessments, create plans, advise, and consult with the interdisciplinary team. You will deliver excellent patient outcomes along a path of recovery and rehabilitation with a goal of successful return to work, function, and activity.
Provide in-clinic and virtual treatments. You will complete functional assessments and develop customized therapeutic exercise programming to increase range of motion, strength, flexibility, and functionality. You will foster trusting and reliable relationships with both patients and rehab providers, maintaining a strong and professional line of communication.
You will participate in team meetings and case reviews.
You will perform regular, timely charting and create exercise home programs for patients.
Required Length and Type of Experience
What you bring:
You have a minimum of 8 years of hands-on experience demonstrated through strong clinical practice, ideally in an acute and chronic rehabilitative care setting
You have a strong understanding of medical conditions, chronic disease management, and knowledge of appropriate exercise prescription and progressions
You are dedicated to treating a range of patients following best current practices as part of a true interdisciplinary team
You are a continuous learner, seeking out the latest treatment and clinical skills to grow as a practitioner and increase the ways you can serve your patients

MINIMUM QUALIFICATIONS:
Education, Knowledge, Skills and Abilities
Bachelor’s degree in Kinesiology and/or Exercise Sciences (asset)

Minimum of 8 years of experience.
Required Physical and Environmental Demands
Must be able to stand/walk for long periods of time and ability to help patients stand and walk. Will require some lifting, bending, twisting of upper extremity to help limited patients. Must be able to adapt to different room temperatures in exercise facility and patient rooms.
SCOPE & OVERVIEW:
Assess patients using evidence-based methods to create individualized rehabilitation program plans.
Collaborate with a large interdisciplinary team to carry out movement screening and formulating exercise program plans for patients who may have acute/chronic injuries, or those requiring guidance around chronic disease prevention.
Record rehabilitation and strength programs in electronic medical records system (EPIC).
Attend team meetings with allied care providers to discuss patient progress, philosophies and share ideas
Ongoing communication with all care providers, as needed.
Develop and Instruct training programs for:
Patients returning to activity post injury.
This may require adapting to the patient’s needs based on restrictions or limitations that the rehab provider has given.
Patients getting stronger, post injury. This may include improving mobility and function to help rehabilitate the patient, minimize pain, and prevent reoccurrence of future injury.
Creating strength and mobility programs for patient’s post-surgery.
Patients who require a transition from rehabilitation and therapy, to training.
Help to keep the exercise environment in good condition, clean and tidy
